South Bergen Jointure Commission
Welcome to the 17-18 academic school year. I wanted to take a minute to update your department on a few minor, but necessary, changes we’ve made at the South Bergen Jointure Commission.
Moving forward, we will no longer be using the fax in order to process any and all Related Services requests. We have created the email email@example.com to replace the fax. We ask that all paperwork now be sent to this email address, as this will expedite all student evaluations and service requests.
We have also updated and made changes to our request forms. We’ve made sure to include more fields for your district to fill out i.e., Case Manager phone and email contact, Parent/Guardian name, phone, and address, as well as the style of form. We ask that you please give electronic copies of these forms to all of your case managers since we will no longer be processing requests made on the old forms starting September 1, 2016. In addition, all of these forms can be found below.
Also, below is a C.S.T. information sheet that we need you to fill out and return at your earliest convenience. We would like to make sure we are up to date on all your current C.S.T. and Supervisor/Director contact info.
